Below are seven important skin changes every clinician should recognize.
1. Hemosiderin Staining

One of the earliest indicators of chronic venous insufficiency is hemosiderin staining. This appears as brown, rust-colored, or reddish discoloration around the lower legs and ankles.
When venous pressure remains elevated over time, red blood cells leak through damaged capillaries into surrounding tissues. As these cells break down, iron-containing hemosiderin is deposited in the skin, creating permanent discoloration.
Why it matters
Hemosiderin staining indicates chronic venous hypertension and should prompt evaluation for:
- Chronic venous insufficiency
- Chronic edema
- Risk of venous leg ulcer development

2. Fibrosis

Fibrosis occurs when chronic inflammation causes excess collagen deposition within the tissues. The skin becomes:
- Firm

Patients often describe the affected area as feeling “hard” or “tight.”

Fibrosis decreases tissue flexibility, impairs lymphatic transport, and reduces oxygen delivery to the skin, increasing the likelihood of skin breakdown and delayed wound healing.
3. Hyperkeratosis

Hyperkeratosis refers to excessive buildup of keratin on the skin surface, producing thick, rough, or scaly plaques.
It commonly develops in patients with:
- Chronic lymphedema
- Longstanding venous disease
- Severe edema
Why it matters
The thickened skin can:
- Trap bacteria
- Increase infection risk
- Crack easily
- Make skin hygiene more difficult
Treatment typically includes edema reduction, compression therapy, moisturization, and careful skin care.
4. Dry, Cracked Skin (Xerosis)

Dry skin is more than an inconvenienceโit represents a compromised skin barrier.
Repeated stretching from swelling reduces the skin’s ability to retain moisture, making it vulnerable to:
- Fissures
- Bacterial entry
- Cellulitis
- Delayed healing

5. Papillomatosis

Papillomatosis appears as small wart-like projections or cobblestone changes in the skin.
These growths develop from prolonged lymphatic congestion and chronic inflammation.
Why it matters
Papillomatosis indicates advanced lymphatic dysfunction and increases the risk of:
- Infection
- Skin injury
- Hygiene difficulties
It is frequently seen in advanced untreated lymphedema.
6. Lipodermatosclerosis

Lipodermatosclerosis is characterized by inflammation and fibrosis of the skin and subcutaneous tissue caused by chronic venous disease.
Common findings include:
- Tight, hardened skin
- Pain and tenderness
- Red or brown discoloration
- “Inverted champagne bottle” appearance of the lower leg
Why it matters
This condition significantly increases the risk of venous ulceration and often requires aggressive edema management and compression therapy.
7. Positive Stemmer Sign

A positive Stemmer Sign occurs when the clinician is unable to pinch and lift the skin at the base of the second toe (or finger).
Although not a skin condition itself, it is one of the most recognized clinical indicators of lymphedema.
Why it matters
A positive Stemmer Sign suggests impaired lymphatic transport and should prompt further evaluation and referral for lymphedema management.
